How It Works. What To Do.
We’re a qualified out-of-network provider for muscular and soft tissue pain or injury (acute, chronic, or recurring). This means we don’t accept copays or process insurance payments, but you can apply for reimbursement after paying us. This could be a full or partial repayment—depending on your group policy.
This works for most PPO policies, Medicare (Part C/D), and can also be used for Auto Accident or Workman’s Compensation situations, too.
This does not apply to HMO, Medicaid, or Medicare Part A policies.
This is not recommended if you only want to repay one session, as it requires some time and effort. It’s not hard, but you need to get a referral from your doctor, physical therapist, or another healthcare expert in your network.
We recommend starting with a telehealth call, which allows you to secure an appointment online in just days, rather than weeks.
Please review the steps below. Once completed, you can complete the SUPERBILL Request Form (see below), which we use to create the Superbill for you to send in.
How To Get Approved
Step 1 — Contact Your Doctor
All of our services are eligible for coverage if you get a referral from a doctor, PT, or chiropractor with a proper ICD-10 code (Diagnostic Code). Just explain why you need our therapy, and if they agree, they will write a document called a referral. They usually will want to fax the referral to our HIPAA Fax line. They may also request our NPI Number by phone or telehealth call:

Step 2 — Send Us The Referral
If they do not fax the referral directly to us, and you receive the referral document, please email the basic information within the referral [tap here].
Step 3 — Fill Out Super Bill Request Form
Once you have their referral, you can complete our Superbill form. Just complete the form below, including all of your insurance group number details etc.
At the end of each month, we will send out a consolidated Superbill of all of the therapies that you have prepaid.
Step 4 — Send In Super Bill
You need to file the Superbill with your carrier (we cannot). We recommend sending in a superbill after the very first session to the insurance company (usually a portal with a superbill link). They will need to approve it before payment, so it's best to get the first one approved before you end up being rejected after many sessions.
If you are denied, we will obtain a letter from the carrier and assist you in resolving the issue.
Our Treatment Codes.
For informational purposes, here is a guide of the CPT codes we can offer:
CPT 97124 – Thai Sport Massage. Massage therapy techniques to increase circulation, promote tissue relaxation in the muscles, relieve minor pain, and maintain musculoskeletal (MSK) wellness.
CPT 97140 – Neuromuscular Therapy. Advanced therapy for acute and chronic pain. Provides soft tissue manipulation, joint mobilization, trigger point therapy, and myofascial release.
CPT 97140 – Prehab Therapy. Advanced therapy for muscular tension, movement dysfunction, and posture correction that contributes to pain and poor performance. Uses soft tissue manipulation, joint mobilization, trigger point therapy, myofascial release, and PNF techniques. Treatment is targeted by an MSK diagnosis using Phy Body Analysis.
CPT 97112 – Neuromuscular Re-education. Advanced techniques for chronic pain, injury and surgery recovery. Uses neuromuscular re-education of movement, balance, coordination, kinesthetic sense, and proprioception.
